About Hospice Care

Hospice Eligibility:

Patients diagnosed with a life-limiting illness, having a prognosis of six months or less if the disease runs its normal course are eligible for hospice. Patients may be referred to Grateful Hospice by primary care physician, other health care professionals, family or by the patients themselves.
