About Hospice Care

Hospice Eligibility:

Patients diagnosed with a life-limiting illness, having a prognosis of six months or less if the disease runs its normal course are eligible for hospice. Patients may be referred to Grateful Hospice by primary care physician, other health care professionals, family or by the patients themselves.

Medications related to the hospice diagnosis for pain and symptom control

The goal of hospice is to care for you in your own home so that you are receiving the highest quality care where you live, with your normal surroundings. Hospice can allow you to continue with your daily activities but with additional support, comfort and relief from pain.

Each patient’s comprehensive care team is specific to his/her needs and the care plan is always evolving to adjust to the needs of each patient. Your personal physician can always continue to be involved while you are on hospice.
